Turn on two-factor authentication for your Apple Account
Quick answer
Click your name at the top of System Settings, then Sign-In & Security, and look at Two-Factor Authentication. If it isn't on yet, turn it on there and follow the steps.
Why it happens
With two-factor authentication, a new device needs your password and a six-digit code that only appears on your own devices or trusted phone number. So someone who learns your password still can't get in. Most Apple Accounts already have it on, which is why the setting often just shows that it's on.
How to fix it
Open the Apple menu, choose System Settings and click your name at the top of the sidebar.
Click Sign-In & Security and look at the Two-Factor Authentication row.

If it already shows On, your account is protected and there's nothing more to do. If it's off, click the button to turn it on, such as Turn On Two-Factor Authentication, and follow the steps.
Enter a phone number you can always reach, choose a text or a call, and type the code Apple sends. That number becomes your trusted phone number.
Still not fixed?
Check that your trusted phone numbers are still yours. In Sign-In & Security, click Two-Factor Authentication to see them, add a second number you can rely on, and remove old ones.

Got a sign-in request with a map that you didn't start? Click or tap Don't Allow. Never give the code to anyone, even someone on the phone saying they're from Apple. The map shows a rough area based on the network, not an exact place.
If you think someone knows your password, change it now in Sign-In & Security, then Password. You'll need your Mac login password.

If your iPhone was lost or stolen, mark it as lost in Find My from another Apple device or at iCloud.com, then change your Apple Account password from a device you trust.
If the codes stop arriving, use the option on the code screen to get one by text or phone call to your trusted number.
If turning it on fails with an error, or you think someone else is using your account, contact Apple Support. They'll take you through Apple's own steps to secure it, and they'll never ask for your password or code.
